How to Report Concerns to the BAT

Report all incidents and concerns that you have regarding an individual's behavior to (to be determined), including as much detail as possible about the behavior/incident. Use your judgment as to what is reportable, erring on the side of over-reporting if in doubt.

Reporting Information

While anonymous reports are accepted, you are STRONGLY encouraged to provide your name and contact information in order to allow us to most effectively and efficiently address the occurrence. Each witness should complete his/her own copy of this report.
